Dear Ms. Farr:
I have telephoned you on two occasions at 317-276-2000 and have not received an answer back. (This is criteria for the University of Maryland to call the police and have you picked up for an Emergency Evaluation.)
You have sent me a form for Release of Information which I will gladly give you if and only if I get a copy of whatever you are able to derive. Please be aware that the University of Maryland may take some months to reply.
Along with the weight gain, depression, and diarrhea with Zyprexa was urinary incontinence. Enclosed is a printout from my website with portions relating to Zyprexa highlighted in red. Most informative as to Zyprexa is the entry for November 15, 2001. Please do not also miss the entry titled "Bipolar Realities" (enclosed). The latter was used to justify crippling doses of neuroleptics during my hospitalization of 2001.
Then there is enclosed the letter from the U.S. Department of Justice to the Civil Rights Division of the Department of Health and Human Services. Major contentions were the falsification of medical records.
Then there is a copy of two pages from a book "Diabetes Mellitus" by Lilly Corporation many years ago that was helpful when I was studying Biochemistry. I showed it to the professor who said he was going to contact your company at the time. I also showed it to the Secretary of Public Safety here in Maryland to illustrate how a "system" could work. Were it not for these two pages, which elucidated Carbohydrate Metabolism, I would be extremely upset with your company. (I found the book in the trashcan of a medical student departing from the City of Philadelphia.)
Please send me a revised Release of Information covering the Adult Day Hospital (22 South Greene Street), the University of Maryland Outpatient Clinic (701 West Pratt Street) and the University of Maryland Inpatient Ward (22 South Greene Street) to the extent I refused Zyprexa even though it may have been indicated at the time. Do not overlook lab results, nursing notations or social worker notes. All zipcodes are 21201.
