Intake/Triage Assessment - P. Tyler, RN
Dear Ms. Ford:
The "Intake/Triage Assessment" by P. Tyler was done at 9:40 pm. That means I hadn't had solid food since 4:00 am the previous day. A span of 15:40 hours.
The allergy to Benadryl is reported at the outset, contrary to the Discharge Summary prepared by Jennifer Oldham, M.D.
If, in fact, I was "labile, disorganized and agitated", I would not have been able to provide intake data nor bloods, nor blood pressure, nor respiration.
The Medications on Intake are falsified. Neurontin and Zyprexa were discontinued as explained elsewhere in the record.
Loxapine was a PRN, as Doris Balis well knew. Pamelor is omitted, as Doris Balis well knew.
At 9:40 pm, without food for 15 hours, I was in no mood to draw pentagrams for a "mini- mental status".
At 3:10 pm, 6 hours prior, I asked the head of the ER what medical school he went to. He replied, "University of Maryland". I thus thereby "walked away from staff".
At 9:40 pm, I could not have "walked away from staff", I was being guarded by University of Maryland Police. I could not have walked or even shifted from position in the lounge chair they provide.
I stated I would not take any medication because I distrusted "staff". This distrust was, an you can surmise, well-founded. (See my comments on Neurontin and Risperdal.)
P. Tyler stated that "Patient is psychotic" because Doris Balis told her to do so. P. Tyler was not able to make a differential diagnosis of hunger or psychosis.
As for "contraband" search, this was done at 3 pm. I watched the University Police examine my two packs of cigarettes while the rest of my belongings were locked up. I told him to, "put them back", which he did. If the cigarettes were Newport or Marlboro, they would have disappeared into the emergency room. Instead, they were Vegas.
P. Tyler heard from Doris Balis about, "non-compliant with meds x 4 months". Edward Bolgiano gave me the same story in his brief interview with me in the presence of social worker Cerise.
P. Tyler is apparently ignorant of the fact I had credit cards and a bank card. These were not taken. My credit line at that point was probably $20,000. If I were dead, these also would have disappeared into the emergency room. P. Tyler was well aware of what happens to the credit cards of the dead.
"Hygiene is poor," comes from Doris Balis. After I walked home the day before, I took a bath and washed my hair. I was wearing fresh "scrubs". P. Tyler is not speaking on direct observation, but hearsay and gossip.
"Drugs/Alcohol" also comes from Doris Balis. I hadn't had a drink in almost two weeks. Dwain Howard, high school graduate from Harbor City Unlimited testified to that before Jennifer Oldham, M.D. in her Discharge Summary.
Referrals: I didn't eat anything until 10:30 pm that night and it was only by being "vociferous". I never saw a Fr. Bill.
As for P. Tyler, if I see him or her again, I will be inappropriate. She relied on gossip, hearsay, and speculation filling out the rest of the report. That was the "fodder" for the first year resident on the ward. I never had so much as eye contact with Angela Udebiuwa until after I was given crippling doses of the neuroleptic loxapine. Angela Udebiuwa was relying on first year resident Jennifer Oldham, if not a medical student, for her information.

is "full of (sh)it".
